COMMUNITY HEALTH WORKER

The community health worker is an integral member of the health care team who has the knowledge and skills to provide effective and appropriate support and services to individuals and families within the community.  Community health workers often visit homeless shelters, coup kitchens, clinics and various other health care facilities.
